We are celebrating Mother's Day in primary this week so I made this questionnaire to pass out to the kids during singing time (or maybe class time) and thought I would share it.
I know there's a ton online but I made my own because I plan on photocopying it at the church (black and white) and wanted to slightly conserve paper.... this is what I came up with :)
